1. I, the undersigned, wish to participate in the investor training exercise and alien visitation (hereinafter known as “the Game”) organized by the Institute for the Study of Extraterrestrial Visitations (hereinafter known as “ISETV”). I recognize and understand that participating in the Game involves running certain risks. This risks include, but are no limited to: injury or death to myself; damage or destruction of my personal property or the personal property of others; damage or destruction of the vehicle which I am driving or other vehicles in the world; abduction and/or dissection of my corporal body by extraterrestrial beings; and persecution or prosecution by federal, state, or local authorities. _____
2. Despite these and other risks, and despite fully understanding these risks, I wish to participate in the Game and assume full responsibility for any damages and liabilities connected with or resulting from my participation in the Game. I hereby release ISETV and all of its constituent members from any and all such liability and I understand this release shall be binding upon my estate, heirs, my representatives and assigns. _____
3. I promise to obey all federal, state, and local laws and ordinances during my participation in the Game. I understand that ISETV does not condone illegal behavior and assumes no responsibility for investors who commit any action in violation of any federal, state, or local laws and ordinances during their participation in the Game. _____
4. I understand and agree that the Game will involve an extensive amount of driving, and that reckless driving and/or excessive speed are not necessary to complete the Game successfully, and that such actions are strongly discouraged by ISETV. I further understand that any accident, injury, and/or damage that arises out of my driving or the driving of any other participant in the Game, whether reckless or otherwise, is not the responsibility of ISETV, and I hereby release ISETV and all of its constituent members from any and all liability for any such accidents, injuries, or damages and I understand that this release shall be binding upon my estate, heirs, my representatives and assigns. _____
5. I have read this waiver of liability and assumption of risk carefully, and understand that by signing below I am agreeing, on behalf of myself, my estate, my heirs and my assigns not to sue ISETV or any of its constituent members for any property damage or any injury, including death, resulting from my playing the Game. I intend to be fully bound by this agreement. _____
By virtue of my signature I acknowledge and agree to all terms and conditions set forth on this form.
